About Our Department
Older adults may present with frailty, falls, poor balance, walking difficulty, memory change, osteoporosis, neurological illness or complications from multiple chronic conditions. Evaluation reviews symptoms, medicines, cognition, strength, gait and the person's ability to manage daily activities, with tests or specialist input as required. Pathways can include medication adjustment, treatment of underlying disease, two to eight-week rehabilitation programmes, mobility support and caregiver training. The clinic aims to preserve independence, reduce preventable decline and help patients remain safe and engaged in everyday life.
